Thank you for sending in your lenses!  We look forward to receiving them.  In an effort to streamline the Simmodding process while your lenses are at our facility, please fill in the Lens Intake Form below with all the information we require.  Shipping directions and instructions will follow once you submit your form.  A copy of this form will be sent to the email entered in the form.  Please check your "junk" folder if you do not see it in your inbox.

Please read some important rules below:

1. We do not accept dropped shipment of lenses coming directly from a seller.  You must take delivery of your lens before sending it to us for modding.

2. Please include this form with your shipment.

3. Please include the serial number of every lens you are sending.  Failure to include a serial number will result in delays.

4. Per the disclaimer on this form, you agree that turnaround time is not guaranteed.  Lenses will be worked on a first-come-first-serve basis.  In some instances, delays could be due to: parts shortages, Covid-19 workplace restrictions, delivery, etc.

Once your form is submitted, you will be given instructions on where to send your lenses.  If you reside outside of Canada, please DO NOT send lenses to our Canada office.  All shipments from outside of Canada must go to our Detroit office.  If you live in Canada, you are welcome to ship to our Canadian office.
